Important Announcement Regarding the Online Execution of Internship Procedures

Due to the COVID-19 pandemic, internship procedures for our students who will undertake an internship will be conducted remotely for a temporary period according to the procedure below. You must download the documents required for the internship specified in the ‘’Internship Procedures ’’ module on the department web page and fill them out as requested. You must complete these procedures at least 5 days prior to the internship start date. After obtaining verbal approval from the department internship committee for the company where you will undertake your internship (Click here to obtain verbal approval.), it will be sufficient to scan your documents into PDF format ensuring that the company stamp and authorized signature on the forms are complete, and send them to the address yildizmatmuhstaj@gmail.com by •creating a new folder and writing your student number, name, and surname as the folder name (e.g.: 19052001_cahit arf). Procedures of those with missing documents will not be taken into consideration. •Your complete internship documents submitted to the internship committee will be delivered to the insurance unit by the department. •After insurance procedures are completed, these documents will be sent to you by e-mail by the department Student Affairs. At the end of the internship, the documents specified below must also be added along with the internship report you prepared. -Internship Evaluation Form (Must strictly be filled out by the Company at the End of the Internship.) -FR-0286-Staj Sicil Formu.doc - The internship report must be prepared in A4 size in booklet form (spiral-bound or paperback, etc.). • On the prepared internship report, company stamp and signature must be applied for each page. • You can send your prepared internship report and relevant documents in a complete manner (Internship Evaluation Form, Internship record form) to our Department Head Office by courier as much as possible.
